- Description of Service:
Phase II Cardiac Rehabilitation is a program of medically supervised and monitored exercise, education and support. Qualifying diagnoses include: * Coronary Artery Bypass or Heart Valve Surgery * Coronary Angioplasty * Acute Myocardial Infarction * Stable Angina with Imparied Functional Ability.
